I start with the lateral cheek as this provides the greatest lift and also we can establish the all important bizygomatic (highest point of the cheekbone) width. In order to provide an even and well blended effect i then inject into the zygomaticus (cheek) major. This also plays another key role in myomodulation (manipulation of muscle activity) as it effectively raises the points from which the muscles contract thus increasing the lift on the corner of the mouth. Moving onto to the defining bd augmentation of the jawline, we injected at the gonial angle (the corner of the jawline near the ear) around 1cm diagonal across on the mandible to provide that aesthetic ideal shape. A point to note is that the gonial angle should be 30% less then that of the bizygomatic measure. The chin was receded so we injected at an angle into the menton and pogonion to lengthen the chin and project it. As we age we lost fat and tissue in the face including the lip area. Lips are the most technically and artistically challenging part of complex facial rejuvenation through non surgical fillers. There has been an epidemic in the ‘duck’ lip in recent times, this is due to a lack of artistic flare and technical ability of some aesthetic technicians. What we are seeing is the vermillion border (lip edge) all to often being over filled which then results in the filler bursting into the surrounding tissues. This results in the lip curling up and and projecting which ultimately disrespects the natural proportions of the facial aesthetic and anatomy thus making the patient have the dreaded unnatural duck lip. To avoid this I prefer to inject just below the vermillion border, this allows space for any migration of the product. Correct placement of the product is another factor to creating a natural and contoured lip and achieving a defined boarder. If volume has been added to the lips, this results then in more projection, if the lip is more projected, we must then consider the impact this has on other areas especially the chin. If the lip now projects further then the chin, this will look imbalanced so I also advise the patient to be aware of the correct facial proportions and golden ratio of the face and consider projecting the chin if this is required to respect the aesthetic ideal that is based on factual geometry and mathematics. I also like to save a little bit of filler, normally .1 to .2ml depending on swelling to do any touch ups at no extra cost incase of product migration. Always ensure you go to an experienced injector who understands and respects the physiology and anatomy. If an injectors guarantees you they will not bruise you then that should be your cue to run out of the door. Suffice to say this is an over promise and not a good start in a consultation.
